ErecAid Supplemental: Effective Erectile Dysfunction Treatment

Plethora Solutions Holdings PLC (“Plethora”, AIM: PLE), the specialist developer of products for the treatment and management of urological disorders, announces results of a study confirming the role its product, ErecAid, as a supplemental first line treatment for erectile dysfunction (ED). ErecAid is marketed through Plethora’s US subsidiary, Timm Medical Technologies, Inc. (“Timm Medical”, Minneapolis, MN).

The phosphodiesterase five inhibitors (PDE5i) such as Viagra and Cialis have become a major resource in the management of ED. Unfortunately, 30-50% of men report inadequate results with these oral medications. When patients fail to respond to the oral medications, physicians are often forced to consider more invasive and more complicated second line agents such as penile injections or urethral suppositories.

New data from a multi-center clinical trial led by investigators at Johns Hopkins School of Medicine demonstrate that use of the ErecAid vacuum erection device to augment PDE5i therapy can greatly improve patient satisfaction. Results of the study were presented at a recent North Central Sectional meeting of the American Urological Association.

Investigators at four separate study sites evaluated 69 men aged 36 to 82 with ED of diverse etiology who had responded inadequately to PDE5i therapy. Study subjects were allowed to continue use of PDE5i’s but were instructed to use the ErecAid device as an adjunct to their current oral medication. Study subjects were assessed using three validated measurements of erectile function and sexual satisfaction: the International Index of Erectile Function (IIEF-5), the Sexual Encounter Profile (SEP-2 and SEP-3) and the Global Patient Assessment Scale (GAPS).

After 4 weeks of supplemental treatment with ErecAid, the IIEF-5 score improved substantially from a baseline of 9.0 to 17.6 (p<0.001). Moreover, of the 34 subjects unable to achieve an erection satisfactory for intercourse at baseline (SEP-2 response of “no”), 27 subjects (79%) reported erections satisfactory for intercourse following the addition of ErecAid (p<0.001). Finally, of 42 subjects reporting no or slight response to PDE5i at baseline (GAPS responses of “not at all” or “slightly”), 31 (74%) reported moderate or great improvement (GAPS responses of “moderately” or “greatly”) at the end of the 4 week study (p<0.001).

According to Dr. Arthur Burnett, Professor of Urology at Johns Hopkins School of Medicine and the lead investigator of this trial, “This study confirms an earlier trial that many patients can achieve excellent results using ErecAid as an addition to oral PDE5i therapy. Based upon these results, I would encourage physicians treating ED to consider the importance of this approach in their treatment protocols and to discuss this option with their patients.”

Back pain treatment could solve erection problems

Men who experience back pain and lower spine problems could also experience problems with erections. Irwin Goldstein, M.D., medical director of Alvarado Hospital’s Sexual Medicine Program—the first hospital-based sexual medicine program in the U.S.—and editor-in-chief of The Journal of Sexual Medicine, recently discovered that treating one patient’s back pain also cured his erection problems.

The patient had experienced a car accident, and complained of leg numbness, back pain and erectile dysfunction. “After completing the initial workup, we determined the problem was the patient’s pelvic floor muscles,” Dr. Goldstein said. “We referred him to one of the hospital’s physical therapist who had advanced training in pelvic floor rehabilitation.” Treating the back pain resulted in improvement in erectile dysfunction.

Erectile dysfunction drugs may not always be needed. Back pain treatment could improve erectile dysfunction long-term. Back injuries can cause trapping of blood. Men with back pain may be experiencing blood flow difficulties in the pelvis as the cause of erection problems. According to Dr. Goldstein, pelvic muscles can become weak and spasm from back injuries, resulting in decreased sexual function from limited blood flow to the penis when muscles spasm.

Muscle energy, core and lumbar strengthening exercises to align the pelvis and relax the pelvic floor muscles, performed by Physical therapist and pelvic floor specialist Kerri Krebs, facilitated recovery. Following treatment, erectile function and leg numbness improved for the patient, who had seen several other physicians and also had been prescribed erectile dysfunction medication.

Orthopedic spine specialist Ramin Raiszadeh, M.D warns however that men with back or pelvic pain, or who experience urinary incontinence need to see a specialist as soon as possible. Erection problems can be caused by …”other medical conditions, including cardiovascular disease, diabetes, infection, inflammation and tumors can cause these symptoms as well.”

Other approaches to treat erectile dysfunction linked to back pain and injury include Yoga, acupuncture, steroid injections, or massage. Disc herniation, muscle spasm from inactivity and weakness, spinal stenosis, and traumatic back injuries can also cause erection difficulties. Keeping your back in shape, and getting treatment for back pain could lead to improved sexual function and less difficulty with erections, and should be explored after more serious causes of erectile dysfunction are ruled out. “If the sexual problem persists, the patient should see a sexual medicine specialist,” says Dr. Goldstein.

It’s treatment time for my prostate cancer

If you’ve been reading my blog for the past several months, you know I have prostate cancer. I was diagnosed with the disease in September, 2008. My urologist told me it was lower-grade and confined to the prostate. That’s what you want, because the cure rate is promising when the tumor is confined to the prostate.  I was put on active surveillance for two years. Just a few weeks back, after a third biopsy, my doctor told me the grade of cancer went up a notch. The Gleason score went from 3+3 to 3+4. That’s a fancy way of saying my cancer has become more aggressive. My PSA had also been tracking up….to 8. Bottom line: My doctor wants me to start treatment. Fortunately,  I have a choice of surgery (removal of the prostate), external beam radiation or radiation seeds. I need to spend the next several weeks, with my family,  finding out what treatment is best for me. Each treatment has side effects. I will schedule an appointment with a radiation specialist.  As soon as I pick a treatment, I’ll let you know. I’ll also let you know how the treatment went.  For all you guys out there, get to the doctor and get screened for prostate cancer. The aim is to catch that cancer while it’s still within the prostate.
